Forms 80 and 1221 for partner visas
For some applicants, the Department requests Form 80 and/or Form 1221 — detailed personal particulars covering your addresses, travel history, employment and background.
Why they catch people out
They're long, detail-heavy, and — crucially — cross-checked against everything else you've provided. A date or address that doesn't match your other forms can create exactly the kind of inconsistency that raises doubt.
How to get them right
- Use your own records (passport stamps, old leases, CV) rather than memory.
- Make sure every date, address and place matches your other forms and statements.
- Take your time — accuracy matters more than speed here.
These forms are really a consistency test as much as an information request. Treat them with the same care as the rest of your application.
Takeaway: If asked for Form 80 or 1221, prioritise accuracy and consistency with everything else you've lodged.
